SIN CHEW DAILY EDUCATION FUND 2024 – APPLICATION DOCUMENTS SUBMISSION CHECKLIST

1. 电子申请表格(共20页) Online Printed Application Form (20 Pages).
2. 身分证 (双面) Identity Card (both sides).
3. 报生纸 Birth Certificate.
4.
  1. 马来西亚教育文凭 (SPM Certificate/ Result Slip);
  2. 马来西亚高级教育文凭 (STPM Certificate/ Result Slip);
  3. 独中统考 (UEC Certificate/ Result Slip);
  4. A 水平 (A Level)
  5. 其他学历资格 Other Academic Qualifications (Foundation/ Diploma Certificate/ Final Result Slip)
5. 曾获奖项奖状 Academic Awards Certificate (Maximum 3)
6. 课外活动奖状 Extra-Curricular Activities Certificate (Maximum 3 of each type) :
团体/ 学会 Associations/ Societies/ Clubs;
运动性质 Type of Sports
7. 家长/ *监护人去年总收入Parents/ *Guardian’s Last Year Total Annual Gross Income: RM
  1. 【首选 PREFERRED】家长/ *监护人报税单 EA Form of Applicant’s Parents/ 或 OR
  2. 家长/ *监护人最近3个月薪水单 Latest 3 months Salary Slip of Applicant’s Parents/ *Guardian; 或 OR
  3. 家长/ *监护人自行估税表格 Self-Assessment Form (BE Form) of Applicant’s Parents/ *Guardian 或 OR
  4. 家长/ *监护人薪水核证信 (雇主/国州议员/村长/校长签署) Salary Acknowledgement Letter of Applicant’s Parents/ *Guardian (Certified by Employer/ Member of Parliament or State Assemblymen/ Penghulu/ Headmaster)
8. 最近1个月家庭电费单 Latest month Household Electricity Bill.
9. 最近1个月家庭水费单 Latest month Household Water Bill.
10. 最近一個月房租收據證明或租約 Latest month House Rental Receipt or Tenancy Agreement
11. 若双亲已离婚、或其中一方已逝世或属残障人士，请提供离婚证书、死亡证书或残障证件复印本。
If parents are divorced, one of them has passed away or person of disabilities, please submit a copy of Divorce Certificate, Death Certificate or OKU card.

备注 Remarks:

  1. 第2至第11项的附件须是 “核證文件副本” 。
    The documents attached for Item 2 to Item 11 must be a. “Truly Certified Copy”.

  2. “核證文件副本”可以找学校校长或老師、村长、国州议员、宣誓官或父母的雇主。
    “Truly Certified Copy” can be certified by Headmaster, Teacher, Penghulu, Member of Parliament or State Assemblymen, Commissioner for Oaths or Employer

  3. 以上所有文件（第1至第11项）须连同“申请表格”上载到谷歌云端硬盘，并将文件夹设为Public 。
    The documents (item 1 to 11) together with Application Form must be uploaded into Google Drive, and change folder setting to Public.

* 若家长无收入，负担家计的兄弟姐妹也被列为监护人。
If the parents produce no income, the siblings who giving financial support are considered guardian.
